Convert survey to php to excel

This project simply involves converting various psych surveys over to HTML (currently in word), allowing people to submit the data to a php script for processing and storage in mysql, and allow downloading the tabulated data into an excel document. Surveys will NOT need to be password-protected or have user accounts, each survey will ask for a "study number" at the top, which is a unique identifier for the subject so they can be paired. 10 surveys need to be converted: BES BSRI DEBQ Demographic EAT IES MES RSES SWBS BMI See attached excel data sheets and survey docs. For most of the surveys, there are "scales" printed in red or blue that instruct you how to calculate the score. You should *not* store the entire data set in the db - just the tabulated scores. So if a line item has a scale of A, the number entered is added to all the other line items with the same scale. ## Deliverables There should be only one PHP processing script and it should be built such that it is easy to add more surveys in the future without having to change much if any code. You will provide the code and the code for creating the needed mysql tables. 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
