Create MVC4 Razor View and Controller from EF Model for 1 page pdf form

I am an experienced web developer in ASP.NET, mvc4 and have good knowldege and work experience in Entity Framework and nHibernate. From the proposal, what we understand is like we need to first create a domain model of all the fields present in the pdf file which you have attached. Then we need to create a view as per the pdf(or else if you have any other design) and a controller which maps the domain model to the view page.
