3D Ebook Cover

I am needing a 3d ebook cover designed. I have attached a zip file with two images in it. The blue "seo" ebook cover is how I would like the ecover to look like as far as the 3D effect. The other image that is white and burgandy is how I would like the design but with different colors. Here are more details: - white cover - where she has a burgandy stripe, I want a blue stripe with the same background color as my website www.whoisdaviddutton.com. - At the bottom where she has a black stripe, I would like an orange stripe like my header on my website. Put my name there in white letters. - For the headline of the report, use white lettering, and for now, just use "3 Proven Ways To Generate Local Leads". - I need it to be saved in a psd file so I can make changes to it.
