Lead-generation Web-based Graphic Calculator/Widget

We are looking for help building a webpage/widget (s) which conducts some simple calculations based on user input. This purpose of this page is accept user input, make a few calculations and return the results in an graphically pleasing interactive page - with the ultimate goal of driving lead generation. An good example of the TYPE of final product we are expecting can be seen at: [login to view URL] Examples of the type of interactivity (sliders, charts, etc) we want (we DO NOT like the graphic presentation here -- not refined enough) can be seen at: [login to view URL] To be clear, this is NOT a mortgage calculator - we will reveal what we are calculating and the calculation points and math once we determine who we will be working with. But they are very basic simple (sum, multiply, divide) calculations. As we will be developing this as we go, we dont have full set of requirements at the moment and have built this on a per hour project. You will be expected to handle the technical coding AND provide for user entry and output in a screen that is aesthetically pleasing and refined TO BE CONSIDERED: You must provide at least 2-3 samples of pages/lead - gen widgets you've built that take user data and make calculations and return them in an aesthetically pleasing fashion. NOTE: if you haven't built anything polished like this but want to be considered, you could also find similar suggested types of existing lead generation webpages as provided in examples as a demonstration of where you envision this going.
