Create and manage a customer survey

We are a digital service that connects budding authors with publishers and require the assistance of someone who can put together and manage (send and collate responses) a survey to go out to approximately 2000 customers. Ideally this would involve sending out an email (we have the email addresses) that directs the respondees to a web page where they complete the survey. Alternatively, a well branded and professional looking email with the questions imbedded (using something like Google forms) would be considered.
