We need approval system to make change on our application. There are around 10-14 data entry forms required which should be like an excel sheet where user can enter details of object names, sources, Attach file, etc. In few forms, data needs to be populated on the table layout from the table with user options. Most of the forms would require table layout with Import/Export excel options and also rows can be moved up or down using drag the row and drop it to the nearst row. Need email template with dynamic value population (Email to be sent from the forms for certain statuses), status update and also we need dashboard designer (To put URL, forms, images, etc). Application also requires user profile, Log on screens. By clicking (encoded) the url on email, application should be taken to particular form with data after sign in. Application should support concurrent users. Application should be user friendly with good design. Two themes are required. DB: MySQL/SQL Server
