Program a test based on an algorithm

I have a formula based on 11 variables (questions that someone would answer) that categorizes the respondent into one of four categories. I need someone to program something that can be put into a wordpress website that allows a visit to answer the questions and then see what group they fall into. It is similar to the test on this website: [login to view URL] (Flash required) I have attached the data you need. The B value is the weighting assigned to each variable. The constant is the starting point. Sheet 2 has the four possible outcomes. When someone is slotted into one of the four categories, I want the concluding page to give them some information about their group from a survey we did.
