Simple CakePHP project + HTML/CSS work (Foundation)

I have an existing website, built using the Foundation framework that I need some new pages coded for. It's fairly simple and straightforward and all the elements are pre-cut from PSDs and ready to implement. Part 2 of the project is to build a very simple CakePHP powered submission form capture system. It will literally consist of a few models and a single controller: Models: submissions categories questions answers A user will come to the site to submit their company for an award in a particular category. The questions are the same across all categories and we'll add them directly in the database - no CRUD is necessary and no admin tools are required. This will literally be a tool to generate a form and capture the submissions. Once a submission is made, we need a simple "thank you" email sent to the submitter and another sent to us with the contents of the submission (which will also be stored in the db). I expect this to be 4-5 hours work in total for a good CakePHP guy with CSS skills. *NOTE*: freelancers only, no companies please - state "I have read the description" in your bid and confirm that you are available TODAY/TOMORROW for this work. It needs to be completed before Monday morning UK time.
