PHP Form + image upload + CAPTCHA

I need a form for my suppliers to complete online that will send me the input data in an email. Please see the attached files. I have made the layout, now I need a developer to make it work. The data in the email will need to be plain text. See the example attached. When the supplier uploads a cd cover, I need to have GD images create 2 copies of it. One of 250x250px and another of 100x100px. The original does not need to be kept. The images to be sent as attachments to the email. If this is not possible, then perhaps the email to contain the URL to the images on our server or another better solution if you have a better idea. The following fields need to output text for the email in ucfirst(). Label/Artist Name Title Artist Genre Track Title (all track titles). Composer(s) Finally a captcha question. As ever, I will use the escrow system on GAF for payment.
