Creating an excel spreadsheet, nice format.

Create an Excel Spreadsheet of Server names and applications... I need a nice spreadsheet that has categories as stated below. I am not sure how to go about this, but I want to update the spreadsheet by servername and enter the applications, but there could be additional applications on one server. I am not sure how to input this on a excel spreadsheet that is why I am here. Please make it look nice and neat.
