simple pixel tracking

I want to add conversion tracking to this website [login to view URL] It is an email form, that submits data to salesforce. I want php/JS added so that the # of forms submitted is stored in a db. Requirements: 1) Salesforce form submission must still work - i think that just means the form part has to stay intact. 2) Track number of form submissions per page 3) System has to allow me to add/edit/remove existing pages. Basically, if i want to modify current page, i just create new page, set URL and it now tracks that as well 4) data in database must know # of conversions, page url and date of conversion 5) ideally # of page visits can be tracked too, then this can calculate conversion rate for each page Please contact with any questions Aaron ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows?
