This Email Survey Campaign project consists of 5 surveys with each survey containing about 100 form fields. Each survey will be distributed via an interanal email campaign software. The requirements for each survey is as follows: (Please refer to the attached doc with the diagrams of the tables - the diagrams only reflect an example of one survey. The other four surveys will follow the same process.)
The Basic Steps are as follows:
1. Use the tables to generate the HTML text that will used by the email campaign software. For survey 1, the HTML text is generated from tables tSurveyEntity and tPortcoGenerate and is stored in the field HtmlText in table tSurveyEnity. The design of the html for Survey 1 is in the attached HTML doc.
2. The email campaign software will then use the HtmlText to send out the email. (See attached HTML file as an example)
3. When the recipient receives the email and submits the form, the information submitted will be stored in the "return" tables. In survey 1, these are tables tPortcoReturn and tSurveyEntityReturn.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

Technologies used should be:
Windows 2000/XP/2003 Server
SQL Server (I think stored procedures should be used to populate the HTML text)
IIS and ASP pages to process the form submissions.
