Description of Sign-Up List: In many occasions for School, Sports, Church and Misc… activities someone always seems to be in charge of organizing what it ordered – offered – in charge of – or signed up for. The person spends many hours making sure that people sign up for things but the people signing up do not really know what is left from a list, that was originally sent out or what is still available to sign up for and often duplicating simple postings instead of knowing exactly what is still required. I want to create a online application for such sign-up sheets utilizing PHP & MySQL. I will try to explain in detail the functions (possibly ‘layout') of all the types of users (Super Admin – List Admin – Participants) Super Admin: Creates Sign-Up lists but once the list is set-up, it is assigned to a ‘List Admin' that does the rest of the lists jobs, but can also be done by the Super Admin. The Super Admin, creates a list which has a unique URL (unique, meaning a URL assigned for that list can never be used again) for the List Admin & Participants. The list created by the ‘Super Admin' as a starting date and ending date. The starting date is the date which you can access the list as a participant and the end date is the date in which the list will no longer be accessible by ‘participants' and have a generic page holder (w/Header|Footer) that says – this list ‘name given' is no longer online, please contact ‘List Admin's' contact information. Super Admin will have a main page in which to see's all the active, or deleted projects and the name, dates all have links to the lists that were made. The Super Admin can also suspend list, erase them, reinstate, reset dates, modify any of the list at any time. The Super Admin when setting up a List has the ‘list Admin's' (Name of Sign-Up list, Super Admin's Name, Address, Phone Number and Email Address) the Super Admin sets up the list and once a ‘list admin' is assigned the list admin gets a email to confirm that they were signed up for a list and they were the ‘list admin'. They have a confirming link in the email and have to click the link to verify they are that person and taken to the page that says confirmed and which becomes there main page to manage the list. They would then get another email that says your status of ‘List Admin” has been approved and to connect again to the unique URL – (the email would state:here is the link, user name and password – assigned automatically and unique, usually first initial/last name and a unique password generator). Each time the ‘list admin' needs to manage the list or review they would be required to put in a user name and password assigned to them by the system. The ‘list admin' then can go to the password protected page with the information the confirming email provided and select from a list of check-boxes in creating the list. - Update or modify the name given to the list given by the Super Admin - Text Header: which is centered in the screen and BOLD and in an editable field. The header text is also the page title.** - Text Body: under the header, which might describe what the list is for or special instructions before displaying the activities or options and in an editable field. The field support HTML** - Text “Thank You” page. This text field that support standard text or HTML which is the text that the List Admin creates to thank the participants for signing up. The default text is “Thank you for signing up for the following items on the ‘'name of list'' sign-up sheet” - List Footer: at the bottom of the page which if used will display more text or HTML –or what every is put in field. This might be a sending off note or special instructions. ** - Create items ‘Participants' can sign up for either by a RADIO button, like if it is for one thing or something you think one person only should get or a ‘Multiples' in which you then have a editable box, once selected, which you could put a quantity in there so multiple “participants' can sign up for it. Also ‘list admin' can select what multiples people could select in – 1 at a time or at least 2 or what every ‘list admin' puts in the box. So if you had 10 items to take, and you had multiples of at least ‘2', then a participant could sign up for at least ‘2' or anything higher. Once the amount is reached, it is closed to sign up for and the then the item would be listed and below each participants name w/email link and how many items they signed up for. The List Admin can select whether to show only name or last name, email address and phone number, but minimum is last name and email/link. So default it would show name w/email linked but if toggled when creating the list might also show phone number next to each ‘participant' who signed up and how many of the items or ONE radio button item they signed up for. The list is designed to show no more then 25 items on each page and when a item is closed, meaning enough participants have signed up, then it collapses but if someone clicks on the link item, it opens down and shows who signed up for what. Once items are completely signed up for then no one can sign up unless someone contacts the ‘list admin' to remove or cancel what the person signed up for – so that it becomes available online again. -The participant will get a email confirming what they signed up for (in a list format of everything single or multiples they signed up for) and participant must verify by clicking a LINK given - that verifies this is what they signed up for. The link verifies and takes them to the list and verifies they what they signed up for. Also says displays a note created by the ‘list admin'. ++In the ‘List Admin page is a field that creates the note in the thank you confirmation page, when someone signs up. The items are reserved for the ‘participants' for 30min and if they items the participants signed up for is not confirmed via the email link confirmation, then the items go back open for another participant to sign up for. **Basically a free form text blox which also supports HTML but would be nice if the fields also had a “live html editor” like [login to view URL] or something else you prefer.. The given name of the list appears in the URL after the name of the domain. So anyway, this is a detailed description of the Sign-Up [PHP/SQL] web based application and how the three different users (Super Admin – List Admin – Participants) work and flow. The pages should be laid out in the US Letter size template and initial layout should be simple. As the development is completed I will require the template tightened up. Difficulty is low for a seasoned programmer but will require good communications skills until the online application is completed. Final delivery is considered as verifiable source code of working application and basic installation instructions. I also require a references and how much you are charging per hour. This is required for future modifications or changes as needed, once description for project is completely functional and delivered.
