Please send a PM with your bid Looking for someone to create a software program and website that will be using for various personal identification processes, such as child ID, elder Id, medical Id and other information ECT. The program must have the ability to add pictures per the various forms that will be included when printed the pictures must also be printed. The software program has to store information digitally, on an easy to read form with an emergency information area highlighted to get attention. Also the user must be able to scan his or signature and save it in a document that requires a signature all fillable forms must have a signature section. Some of the forms that will be on the software will include , medical release forms, wills, legal forms, DNR (Do Not Resuscitate) forms, Also must be able to scan other documents such as a medical prescription, fingerprints, medical images such as X-ray, MRIs dentals, birth certificates ect Other information on the software will be the graph lab value medical history, laboratory values, medications, vitamins and supplements, doctors and insurance information, , medical records, blood glucose levels, weight loss, cholesterol levels, and other data, create personal reports, track fitness goals and stores, MRI's and EKG's, financial information and other electronic downloadable forms such as poster. The Software must also prints posters/flyers at the click of a button and documents and save them in their specific section, The software will have a public access records area and a password protected data . The owner will determine what information will be viewed by the public and what information will be restricted. This program will be installed in a portable USB flash disk, card and also on a PC and or a floppy disk and must be universally accessible via all computers and or web portals looking for a software that is user friendly, plug and play and the information form has to be a simple fill in the blanks format and or check the box format. Must be able to update and save information for multiple individuals each having his or her own data. We have some forms that we will provide as samples to be used and forms that we will need to be re-created. Some of these forms will be on the software while some will be on the website for download Again must be able use virtually from any PC. The website will provide a download access with registration key for the product. All of the upgrades to the software will be downloaded from the website. Being that we will be having various updates we are looking for a reliable team to work with to create this software and possibly our website where the program can be downloaded after payment has been approved via paypal and credit gateways.
