IPAD Application to capture consumer contact data

Need IPAD application developed to capture the following contact info on an IPAD: First and Last Name City, State & Zip Phone Nbr If not residing in the US, then ask city and country Email Birthdate mo/da/yr When was the last time you purchased a pair of sunglasses: 0 – 6 months ago 7-12 months ago 1 – 2 years ago More than 2 years What are your top two reasons for purchasing a new pair of sunglasses? Eye Protection New Fashion Trend or Look New Technology Lost or damaged existing glasses What was the last brand of sunglasses that you purchased? Oakley Ray ban Maui Jim Costa Kanaan Designer Brand Other 1. Will capture and store contact data in an offline database eliminating the requirement to be connected to a wi-fi or 3G/4G network. 2. Will allow user to selectively run a report of all contact info captured and not previously exported to create a contacts data file where information captured is sent to an email recipient as a file attached in Excel csv comma delimited format. 3. Offline database will be updated to indicate that contact information has been exported but contact information will not be deleted. In order for contact information to be removed from local IPAD database, user will manually select records to delete one at a time. 4. Offline database will allow user to update records to correct invalid information. 5. Design of IPAD app will adhere to corporate branding, i.e. colors, fonts, logo which will be provided to developer. 6. Provide a tool/process/instructions for installing app on IPAD. IPAD application must not require any run time or license for software to run app on IPAD.
