I need to create a PHP Database But also need to record points in images

Hi I have reviewed your project description and have understood that you basically want a small Web App. In this Web App there would be one form to push the data to database, another form would basically would be a follow up page which will show thanking message or so. The third would be an image of human body which will be connected with the form in one way or another. Please consider that in order to highlight any particular part of the human body, the image shall be an interactive image and not a regular image. We can certainly do that. My bid is a little higher than most people but you neglected to mention anything about security, of course when you're talking about medical records you have to follow certain steps ... obviously I will not do HIPAA compliance for this price ... but it would have some security ... is this supposed to be a public site or just an Intranet or Extranet??? all that affects price, if it's Intranet it would definitely be cheaper ...
