Online PDF Form Filler design / coding

I don't know if this is possible. I have a school webpage using Joomla. I want teachers to be able to login and then write report cards online in the .pdf format. When they are finished filling in the report card, I want the completed .pdf to be emailed to me. I will provide all the content I require on the document and our school's logo, but I want someone to design a highly professional and attractive .pdf document that can be accessed and filled in on our website, then emailed to me when finished. The design of our form is very important to me as parents will also be receiving these report cards. I would expect our logo to be implemented professionally, some type of background watermark design, and various other small details on the page that really makes our report card stand out. Our logo may also be slightly altered by moving the text if so desired for design purposes. This document will only be 1 standard A4 size page.
