Modify Bootstrap Template

This is a straightforward project that involves updating and extending an existing bootstrap template (attached, called Open Mind). Included in this project are 1) Mockups (PDF) 2) Outline (Word Document) 3) Assets ([login to view URL] with image files) 4) Bootstrap template (zip file) Objectives: Mobile Ready. Most critical to this project, all the bootstrap templates/pages must be mobile ready. The goal of this project is to create bootstrap templates that are mobile ready from the wireframes that are provided. There is a lot of pages that are very simple, such as the form entry, and a lot of repetition of code for most of these pages. The most important pages are the schedule appointment, choose medication, and choose diagnosis pages. The reference site shows the style of the site, and existing design. Information Architecture and Design The goal of this project is to have a simple, elegant design, and you are encouraged to improve the design with font choice and element placement. Wireframe Notes Page_001-Page_005: These wire frames show the Locate, Pick, Select Page Page_006: Shows the Enter Birthday Page_007-Page_008: Shows the Reason for a visit page (See the Reference site for this functionality Page_009: Sample Question Page (implemented in the reference site) Page_010: Schedule the appointment page. This page allows a user to select an appointment, and is similiar to zocdoc functionality ([login to view URL]). It is very important that this functionality is mobile ready. Page_011: Short summary page Page_012-Page_013: Form Entry Page, Form Entry Error/Validation Check Page_014/Page_017: Long Summary Page (Page_017 adds one icon, medical history, based on Page_015-Page_016) Page_015: Choose Diagnosis (This functional on a mobile device) Page_016 Choose Meds (This functional on a mobile device) Reference site: [login to view URL] (Use the login link, username:riyaad(at)[login to view URL] password: password) N.B. After entering the username and password a new link will appear (Demo). Demo has four buttons 1) Contact Lenses 2) Glasses 3) Eye Problem 4) Follow Up The Contact Lenses button works, and shows a working model of the process flow this application supports. This is an ongoing project, and we will have more work.
