I need to get a simple Facebook app live on Monday, and need someone to work with my designer to get it tested and working on the day.
She will provide graphics but will only be able to start designing on Monday morning (GMT) so the structure of the app should either be built beforehand or the whole thing will need building from scratch on the day. I will need you to develop on a separate profile, then once signed off it can be moved to the final location where it will be live for only 2-3 weeks.

The app:
- The top section needs to ask people to Like the page
- Next, a video needs to be embedded (from Vimeo)
- Then an entry form needs to be built with these fields: name (free text), email address (free text), answer (up to 4 digit numerical field), Over 18 checkbox.
- Entry button leads to a "thanks" page which will include Facebook share buttons.
You will need to be able to provide me with live access to the data (a dump into Excel?).
