Interactivity inside a video - open to bidding

We need an application/ tool that allows to insert activities, questions, comments or other information inside of an mp4 video so that the user can interact with the content (questions/ surveys), download documents or read information. This application should be able to publish the video with this information.

The information would be taken from a repository and comments that are made during the video should be accessed or acquired by a Learning Management System, so that we know which student made which comment.
