MS Word and PDF form needed from current document

Current MS Word document needs to be changed to allow user to input information. It is currently a form with blanks to fill in after being printed, I need a user to be able to print it as is (to fill out manually) or be able to fill it out in a PDF form and word 2003 and 2007 format. Needs to look exactly as it does now (all blank lines in place etc) but when a user fills it out the lines remain in place and text is placed properly over each line. Most blank areas are multi line so when the user gets to the end of one line the text must NOT change size or position, just go to the next line and continue to fill out the form, again with underlines (blank lines) still in place and NO movement of lines or other form elements. check boxes are required to be checked by user where they are boxes on the Word document now. Most of form is simple fill in the blank, although there are sections that are multi line as described.
NO layout changes are needed and form must be able to be unprotected by me for future edits, but should be protected so users can not change or add to text anywhere except in the proper form places. NO web interface is needed, purely a Word and PDF deliverable.
