Need An Attractive Opt-In Box Designed In the Pregnancy Niche Using Wordpress platform

I am looking for someone to initially design an opt-in box for me in the Pregnancy niche using Wordpress. This is the first of many jobs that I would like to take up so please treat this as a possible ongoing work relationship through [url removed, login to view]

I am looking for someone who is fast, efficient, has impeccable design skills and has worked with Wordpress as well as straight up html sites previously.

You must deliver on time, provide value for your work, have very clear communication, and be willing to take on more work once this project has been completed.

The opt-in box should be fully functional in Wordpress and across all major browsers.

I would like the color theme of the site to match the existing site which I will provide to you. I will also provide the text to include in the design.

This initial project is purely for the design of the opt in box and NOT for the slicing and implementation of the template. *****However it will work to your advantage greatly if you can slice, implement and install the opt in box.*****

I would like you to do a mockup of the site as a paid test based on the above factors.

All designs put forward MUST be original designs and NOT pulled from a template area.

The project should be completed in 1 day after project signup.
