Picture Upload Script

What I need is a picture upload system for my website. Attached you will find an html version of the Admin Center and also what the main picture page should look like. On the main picture page it should show the pictures from current month and also have a link to the past few months and a link to see all months. If this link is clicked a page is brought up that has a link to all months in the database. If Id like the pictures to be no bigger than the following sizes:

Thumbnail Size: 100x80 pixels

Full Size: 325 x 262 pixels

Everything else should pretty much be self explanatory when you see the attached files.. The way the pictures are displayed should look like the layout on [url removed, login to view] page. The pictures would be divided up by date here. The user should be able to click the picture to view the full size original and also the details such as date taken, camera, title, etc.. Basically all the data from the input form. You can see what I want at [url removed, login to view] I dont want all the stuff like this week 1 year ago but the layout of how the pictures are sorted by date...

The only part ive not designed is a form or method to allow me to edit pictures or delete them. By edit I mean change the title, date or something like that. Id also like a way for a user to comment on the full size original photo...

The backend should be programmed with PHP and MySQL. My webhost is at [url removed, login to view] if you want to view what they offer to make sure what you design is supported by them... Basically just make it work using the layouts ive included..
