I need this project finished today.

The form on my website is not working properly and I need some one to fix it.

Things that Need to Be Changed on the Website:

1. The form needs to display the names of the fields in chrome, fireox and IE.

2. The Submit button is not displaying.

3. The Form does not display at all on the blog.

4. I need to manually be able to change who receives the form information. I need to be able to add multiple email addresses that it sends to.

5. The top menu needs to be displayed properly even when the language selector is turned off in the Multilanguage plugin settings.
