I need exactly the same with the following options:

1) FLV can be loaded by passing a variable in param name.

2) Logo of the site same like myspace can be loaded by passing a variable in param name.

3) Website Name can be passed in by passing a variable in param name.

4) Send to friend link which will open a php file (You do not need to code the php file just pass on the video id which will be passed in a variable in param name).

Try make it look almost exactly like that a little different to not eaise a copyright issue. let me know if u have any questions.
