osCommerce / PHP + mysql DB small project

I need a cleanly designed separate PHP page that will sit in the admin area of oscommerce which allows an admin user to input / search / view 9 fields.

These are:

OrderID | Products | Name | Address | Date | Status | Further Comments | Staff

OrderID should be as per orders table

Items (pulled from the orders_products / products_title)

Name: (customer name from orders)

Address (delivery address etc. from orders)

Date (date purchased from the orders table)

Status: (last status in the orders_history)

Further Comments: (last comments in orders_history)

Staff: (new, char 32 for entering staff name)

The page itself should allow an admin person to enter all the above details into the database, so then it can be searched on: orderID, items, name, address, further comments, by date, or staff. Of these, items, further comments, name, address - these should all be wildcard matchable. (e.g. search on 'Sam*' in name could bring up: Samantha Wilkinson, Sam Jones, etc.).

Date searching should be done via an easy JS calendar application between two dates.

Results should be displayed in CSS 'table' format. And the orderID should be a link to a valid web page (so it is clickable and the admin staff can view order details immediately). It should also be possible to update the Status + further comments. Each change in status should be dated, and a history kept - as if they were editing the comments on the [url removed, login to view] page itself. There should be a button the user can press next to each unexpanded status to view the history status/comments for that particular order.

This data is for one admin to make a note of issues, then another one to come along to change/fix these problems. Then, later, it should be possible for someone else to come along and search on the history to see when things were fixed.

Budget is around 300 - 500 dollars. Preferably closer to $300.
