548166 aWeber Scrolling Bar Form

I require a scrolling bar form to allow people to subscribe to my newsletter.

It should be like the one at [url removed, login to view] that scrolls with you as you go down the page.

The raw aWeber code to link to the name, email and submit features will be emailed to you if you are awarded the project.

And use this image on the left of the form:

The bold brick red header text should read "Get Your Free Audio Lesson on the Power of the Pattern Interrupt" and the text on the "submit" button should be "free instant access"

The form should scroll smoothly, and in the top right there should be an (X) to remove the form.

The background color should be dark blue.
