We are providing for a safari company feedback forms so the clients can fill in a survey after their safari.

The survey will need to be provided in English but with document that can be edited so it can be translated into other languages.

It must feature the logo (attached) and a nice visual design (see sample attached). The following data must be present so clients can give a quantitative evaluation and then enough space for qualitative evaluation (you can use more fancy words than the ones provided here):

General evaluation
Itinerary
Guide
Vehicle
Food
Lunchboxes
Accommodation
How did you know about us
