I need a Thank You / Customer Satisfaction Form flyer designed.

The front will have the thank you message, and the back will be the form to be filled in for the customer satisfaction.

We need the front to be big, colourful and standing out. But with a clear section at the bottom telling you to turn over to the other side for the customer satisfaction survey. The survey itself should be plain and simple, just the wording with a big box for people to put in their thoughts about the company.

Can designs be clean and sleek please. I know I said colour, but that doesn't mean I want it to look like every colour possible was used. It needs to be easy to read and look nice. To assist you I am going to upload two flyers that we also use.

Please note that the space for peoples comments needs to take up between 70-90% of the area on the back of the card as they need to write in it.
