Konkurrence Instruktioner

I am starting a medical clinic, I need a logo for it. I would like it to have the four corners states in an outline in the background (Utah, Colorado, Arizona and New Mexico). I would like the colors of blues and greens. The name of the clinic will be "Four Corners Infusion Clinic". At the bottom of the logo I would like the city and state, "Farmington, NM".

Attached is what the fours states together look like, I would like to see an outline of the states in the background of the logo.
