Awesome... Would I also be able to modify and make simple changes.. for instance if I wanted to delete the Disability on the form... or change from 6 boxes to 3 boxes... or is this something that would need to be done on your end? It really is just 3 variations I wanted of this flyer.

I can send you the original file but making the changes would really depend on your level of comfort in working with the template :) If you know exaclty what variations you want at this point, I can do those for you for a nominal extra fee as well and send them over in the image format that you have asked for.

I like it! Did you see the pdf with my companies Logo? If it didn't attach it can try to email it to you. You could add it on the top and then change the green border to match the blue on the logo. Can we also bring in and shrink the name, school, break time, email and phone number part.
