Website site title displayed at Google not correct

I need someone to tell me how I can fix the following. I will do the fix I just need to know how to fix the below.
I have created an individual page <title> for every on of my pages on my website [login to view URL]
EVERY page title EXCEPT the homepage title is picked up and displayed correctly at Google.
For some reason when I search Google for [login to view URL]
or site: [login to view URL]
the title returned is 'Optimal Web Design' see in Google results [login to view URL]
I do not understand why it is returning this as many months ago I set the <title> to 'Custom Website Design | Australian Web Design Company servicing Sydney, Melbourne' See screenshot [login to view URL]
If I check the most recent cached version of homepage it was cached only a few days ago [login to view URL]
and the title I updated months ago so I dont understand why still returning title at Google as just 'Optimal Web Design'
Provide solution to fix the above issue so Google returns the title I set ''Custom Website Design | Australian Web Design Company servicing Sydney, Melbourne' instead of what it is displaying now ' Optimal Web Design'
