Quick Code Learning Project

I am learning to code and have a simple HTML form which I would like turned into a full-stack web application. The form is only a few fields (name, email, favourite animals) but I need someone to add a MySQL back-end, connect them and add the necessary code to make the form (and a brief response page) function properly. I will then use this full-stack template to add to and learn from as I tweak the HTML, CSS, Javascript and SQL. For the back-end I would like this built using either Python (2.7) or Node.js and for the entire solution to be moved to my home PC (an Apple Mac) so I can continue to add features within the framework you provide. This should take an experienced Python or Node.js WEB-developer around an hour to create and another hour to pass over to me in a 1 to 1 session over Skype or Teamviewer. Further tuition may be requested at a later date. I will want to view resumes or portfolios of previous work you've done and as well as see a listing of what skills you have in advance.
