Build simple order-form tie into osCommerce back-end

I have a customer who wants an order-form style page where their customer can login and select predetermined products with predetermined pricing set for that customer. Probably would be check boxes, radio buttons and/or drop downs for product selection. This form/template must be able to tie into the existing setup osCommerce shopping cart system. All products/pricing and account information for customer will be pulled from that MySQL database for the osCommerce store. Order form must be flexible in that it searches what products are available (that we put into the osCommerce back-end), and their pricing, from the database for that specific user's account. An add-on will be used with osCommerce to create the ability to have multiple customer with each their own product/pricing (already have the add-on needed). No two customers will see the same products/pricing, again, pulled from the database. Needs to obviously have checkout function, using the preset checkout function already built into osCommerce, which will be set to checkout with PayPal. Only thing I need here is a login/pass boxes on the public page and the product order form on the secure page. Entire site is SSL secure. Customer very specific that he did not want a shopping cart. He wants this order form. If I can't product this, project is scrapped. I figure this is a fairly simple project for someone savvy in MySQL and PHP development. Customer is pushing hard, hot demand. Need quick reply and short finish date, as he has a deadline of his own to meet. I run the VPS this will be established on, so access to needed accounts/programs is no problem. Contact me asap.
