Embed a FORM into a template page to grow a DB

I am developing a DUMMY hotel booking site with the following HTML template. Note: it is not a WP template. [login to view URL] When I say dummy, it does not do any search, booking, payment etc at this stage. It's just a placeholder to promote my crowdfunding campaign for the actual site. Now, I need a page in it with a FORM to collect (crowdsource) possible hotel list. I am targeting a very specific kind of hotels and I am going to request people to name them, with some details. The form should contain name (of hotel), place (of hotel) and county (of hotel - drop down menu), address (of hotel), website (of hotel, if any), kind of hotel (I shall provide entires for drop down menu). Users should be able to put a brief description of the hotel too. Then they could press submit button. If one wants to suggest more hotels, form will appear again. Finally this information is collected in a database and should be able to download at least as a CSV file. If you got any more imaginative ideas about DB, please let me know. This page should be able to integrated to my dummy site. The rest of the pages, I do it on my own. I will host it temporarily on my hosting space and you will be given limited access. Or you can put it in your hosting platform and develop and then transfer the site to me. Additional/desirable Functionalities: 1. Each entry by users becomes part of a growing blog/list, which everybody can see. 2. Repetition of entry is blocked by name of the hotel - that is, when a new entry (hotel name) is made by the user, the database checks it against the already existing lDB list and alerts the user (that there is already an entry). If the user chooses to continue (he/she will have some reason), user can create a new entry.
