I require a Vacation Request Form to work with Outlook 2002. I have a template (pst file) of what the form should look like. The form requires customization to require a few additional fields. Download the Outlook 2002 "[url removed, login to view]" template from [url removed, login to view] It contains the have the following forms: Vacation Request Form Approval/Deny Form And a Vacation Time Taken Form The way it works is: Once the user sends the Vacation Request form to the approving manager, the manager receives it with two buttons on the form. One for Approve...and one for Deny. The manager clicks on one of the buttons and it goes back to the originator with the message of Approve or Deny.
