Iphone/android form app with API calls

What I want is a app that is a form of this PDF (in swedish) [login to view URL] ## Deliverables What this program shall do is to make it easy to file a complaint with "v?sttrafik" if the bus or tram is late. To do this today you have to manualy fill in a paper or pdf and (snail)mail it. Sins you have to keep track of the busses and times that were supposed to go at time x his is something people often is to lazy to do. But if they can start to fil in the form on their phone at the minute the bus is late and then simply print the form when they get home more people will take advantage of this. In its basic version all that is needed is a form that collects the information belove, and then creates a printable pdf (or similar format) with the address viable as in the PDF below (the user must be able to print and fold the paper in A4 with the address visible) [login to view URL] The form or the final printout does not have to look like the PDF, just as long as all the information is in the letter the app creates. I should be able to correct things here if I look through the source code. [text feild] = simple text field [date feild] = make a simple way for the user to choose date get a list of the X nearest bus stops if you have a GPS enabled smart phone. [stort textf?lt] = here the suer must be able to write a bit longer. [plane text] = just a headline informational text + transfer should be expandable and preferably be able to add and and remove as needed [from gps api] = maybe the most difficult task. By using API from the buss company you can get the nearest x numbers of bus stops calculated by your GPS position. You are also able to start typing (ajax?) and get suggestions on bus stops with similar spelling. This shuld be default and gps positioning an extra option. This is not an dealbreaker if this cannot be fixed sins the api dev page is in Swedish. But the more you are able to take advantage of the API to make the form easier to fill in the better and more likely you will get the job. The form must the be able to be saved so the user do not have to fill every field the next time.
