Updating a website - Wordpress/Gazette Theme

1) I have the following pages on [login to view URL] : Home, Videos, Forums, My Story, Help Near You on my website. 2) For each state, on the "Help Near You" page, I would like all 50 states mentioned, and then I would like the page to mirror [login to view URL], for every single state where users can find recovery centers directly in each city. This may require tapping into a search on the backend of recovery centers in each major city. The Help Near You page will basically list all 50 states, leading to cities in all 50 states, which lead to recovery and addiction centers in each city. This is a must have. As long as it automatically searches recovery and drug facilities based on zip code or something, that is fine as well, but must be professional looking. 3) The video page will display multiple videos on addiction, with some text. Just post a couple videos here (make a cool video page) 4) The My Story page will display mostly text, with some photos. Nothing to add here. 5) Forums page needs to lead to a login page, where people can sign up to login to forums. I would like this similar to recovery dot orgs forums page where we can see recent discussions and categories of the forum on the home page. Obviously this should lead to a modern looking forum page. I am open for options on this 6) Fix format - as you can see, my logo is yellow and black. I want the overall text and color of the page to mirror [login to view URL] with black background and white text. 7) I need social media buttons on home page where SEARCH is located in top right hand corner, then move search down to be above the right widget that shows popular, latest, comments, tags, subscribe. Social media icons need to be Facebook, Twitter, Google+, Pinterest, LinkedIn, and YouTube, in that order. URLs will be provided. These icons must show on every page along with banner and menus. Please do not bid on this unless you are confident you can do this job. Most importantly, I need to know how this can be edited after you are done when I want to make changes. Need to provide exact clickpaths on how to change so please document for me. Required before payout.
