On the store [login to view URL], I want to add an upsell option that says "we can custom print your name on the spinner for $9" -- have a drop box for "no name" or "add name"

and then have fields for them to enter their first name and last name

If someone selects yes from the drop down menu it adds $9 to the product for custom name. And it also includes the text they entered in the box for the order confirmation so we know what name to print on it.

Very simple project.

I don't care how it gets done I just need it done in the next few hours. If you can do this we can start working together regularly.

UPDATE: I am willing to pay $35 USD for the job at most. I know how simple it is.
