Need a few PHP/MySQL scripts written

I work for a non-profit in Denver, Colorado and I believe that a headache or two I currently have could be fixed easily with a couple php/mysql scripts. Basically I have 40 employees that are required to review two or three pieces of information every week. Because none of them are in the same place at the same time, I would like a way to disseminate the information electronically maybe as a pdf that is presented to them as they enter their user number or name into a prompt. After they read the page, I would like for them to be able to check a box that it was read and understood. The back end database should record simply who checked the boxes and when so I can keep them all in compliance. I would like a supervisor interface where I could run reports and see who is not reading the information as well
