[url removed, login to view] this is the current application people use to join the association. They print it out and then fax it in.. they want this automated in some sort of form to submit the data only (not the money).
All the text on the form will have to be displayed
All the business categories will have to be something that they can select so they can be listed in each one on the website
(each business category they pick will be an additional $10 to the fees of joining)
(page2 of the membership application) they will have to enter all of the same information, select which type of membership they want..( regular member at $95 or member vendor at $190) and then they will electronically sign it with the appropriate legal wording
(page 3 of the membership application) they will have to also enter all this info (if the info is the same, they can just click on a box and it’ll auto fill in for the name address phone number etc. but then they will want to enter a description (500 characters of less)
then they will have to pick other certain options and the form will calculate the dollar amount for them and give a grand total of all three pages listing the amounts by different categories so they can take that to the payment page and use the appropriate buttons.
I need a payment banner to be used for the page, I can send you the png to be used and will need transparent images to be inserted that go with a payment theme

It’s a big form, but forms are easy. The tricky part is the money calculations, and being able to separate it and display it properly.. $100 is assigned for this project max
