Integrate ajax script on dialog box

Task: Integrate ajax call to populate options for multi-select drop down in the script attached
Step 1: Create a user table with four columns (id,profession,city,organization)
Step2: Once the [login to view URL] is loaded the distinct professions from the user table must get loaded in the drop down
Step 3: Once the specific set of professions (more than one profession can be selected),cities corresponding to those professions (ie if there are two users whose profession is web designer and city is New York and Toronto,then New York and Toronto must get loaded in the options of drop down for city) must get loaded in the city drop down
Step 4: Once the cities are selected,organization corresponding to those professions and cities (ie: organization of users working in that profession from those cities) must get loaded in the drop down
Step 5: Once the ok button in the dialog box is clicked,the form should send all the options selected for profession,city and organization to [login to view URL] and the options sent must get printed
Note: In each of the step (ie: from step 2 to step 4 as the user is selecting the option) the number of users who are selected as per the option must get displayed with in the dialog box
The complete script must be sent back with the integrated files and mysql table SQL .
The work will be tested by connecting with the tables on the same data before signing for completion
