Create a Website for capturing replies to some questions and displaying the results on a single page
Coding should be in .php and databases in MySQL.
Please see attached Word Document with further instructions and requirements.

Your task – should you choose to accept it – is to create 2 web pages
1. A web form with questions and blank text fields for replies
2. An Admin page (password protected) to see all the results at one place

The domain name will be Entertainmentfeedback.com. We will provide the FTP access and database/hosting space.
Colour scheme, background, and fonts must strictly follow our Brand guidelines. Please see the Brand Guidelines document (Attachment #3) for guidance
