There should be 2 separate forms one used by admin1 ie dependent form and one used by admin2(doctor) ie credit report generation form . All are connected to each other.
1. Employees will add dependent details, it will be approved by admin1 and report is generated
2. That dependent details are used by doctor to generate credit reference report
