Google Maps heatmap site to report stolen items using PHP MySQL

Okay so the basic idea is to create a heat map on google maps using PHP and MySQL where anybody can come and select a point in the map and report a stolen item anonymously. The system should contain the following features:
1. User should be able to select country and city
2. User should be able to move a pin in the map to set the place where is item was stolen
3. User should be able to describe what was stolen in a text field but its not mandatory
4. Map should open in the current city using GEOIP. If city not available default to country.
GEOIP should be installed locally not use external service.
5. Map should have zoom and scroll capabilities.
6. Design should be responsive.
7. Design for buttons and text fields should be consistent with our website
8. There should be a limit per IP for reporting a stolen item daily, this should be 3 but we should be instructed where to change this, maybe in a settings php file.
9. There should be feedback alerts for users when the report Success, or errors.
10. Users can toggle between heatmaps and dots but default should be heatmap.
11. There should be a strings file where all texts can be changed.
12. A JS event should be fired upon successful stolen item reporting (we will provide the script)
13. Every stolen item should be stored in a MySQL table containing ID, country, city, lat, long, text entered by user, IP
14. There should be a sharing feature where the user can share on facebook just with a click of a button the image of the current heatmap he is viewing whether he picks heatmap or dots. The shared image should link back to the URL where the system is currently at, this URL should link back to the same site also this url should be on the configuration file. This should be on the bottom.
15. There should be another simple share this site button with also a custom image and URL on te TOP.
16. Site must have all facebook meta tags with an image of a heatmap or other we choose.
17. The system should be installed on within our site (database should be separate), which runs on a classified php mysql script called flynax.
18. Well-documented code is needed.
