Generate QR code, Store and Read it

Hi, I have a project which i`m struggling with. I need a QR Code generator for Inventory system that will Generate the code based on either the product name or at random ( making sure it's always unique ), i also need it to be stored in MySQL. After that i need to be able to use mobile or tablet to scan the code which will then populate input box for me to pretty much search for the product in the DB. What i am looking for is already done but it looks my knowledge of editing things to work as they should is limited. I have attached a photo of what i need, it's a simple input box with a barcode button on the side which pressed pulls up a QR/Barcode scanner and then populates the field with whatever was stored in the code
It's a very simple job and shouldn't take longer then 2-3 hours
