Create a Form in the yii framework.

Put a link to this form on the Homage page, to the left of “Search for Catering”. Call it “Request a Quote”

Form should include the following fields, text fields only, all required:

Title: Request a Quote

Order: (Larger box)

Name:

Tail Number:

Airport:

FBO/Handler:

Date / Time:

Phone Number:

E-Mail:

3) After this form is filled out, send inflight@[login to view URL] a simple notification via email. “You have a new quote to review”.

4) Send the user who’s submits this an email:

“We have received you’re quote. Next you will receive an email quote with payment links from our vendors. You may receive multiple quotes. Please select the one you feel is the best.”

5) Create a table in the MYSQL database, and store this data in the database.

6) Maintain the look and feel of the rest of the website - see the contact form.
