I am looking for a web application based on PHP and MySQL. The gist of the project is a simple input form with many fields for data entry. From there, a search index where users can select specific fields to search and matched results listed. On the results page, each record shown will have a link to specifically designed output page. This output page has already been "designed" in the look and feel that is non negotiable as it is for a Government contracted site. The output page will be available for printing, as well as sending as an attachment via email. The last part of the project will be an easy way to edit a record already exisiting in the database.
The fields have already been worked out, and I do have a simple input form already made, but would prefer a more professional job in the way it works, look and feel.
