564769 Modify Form (Easy Fast Job)

Simple Project:

1. Modify the form code of the attached zip file to insert the necessary Mailchimp form code from the code pasted at the bottom of this description to make the form work with my Mailchimp Account. Only insert the needed parts of the form code to make it work, anything else can be stripped out.

2. Modify the graphic at the bottom of the form so that the text "powered by [url removed, login to view]" is removed.

3. Test the form to ensure it posts to Mailchimp successfully and deliver the customized form back to me in a zip file.

Note--The form only has the fields "First Name" and "Email". However the Mailchimp form requires input of "preferred mail type", just hardcode so all form inputs will post as "HTML" for the selection.
