PHP + SQLITE simple custom implementation

This custom implementation needs the following :
- PSD to PHP app (custom simple design)
- 1 login simple form + 1 custom form with about 4 fields.
- Creating the SQLITE db to store all these records (db does not exist now) , developer must create it
- Must work on all major browsers
- Use responsive design
- Codeignither would be a correct approach
The whole project work can take up to 4-6 hours moreless by a skilled php developer
This project is the first part of multiples mods.
At this moment we can supply the PSD designs and small app info guide (simple txt)
